14-1688. Fraudulent signing of credit cards or sales slips; essential elements.
For you to find the defendant guilty of fraudulently signing a [credit card]1 [sales slip or agreement] [as charged in Count __________]2, the state must prove to your satisfaction beyond a reasonable doubt each of the following elements of the crime:
1. The defendant signed a [credit card3]1 [sales slip or agreement3] with a name other than his own name;
2. The defendant was not authorized to use the credit card;
3. The defendant intended to deceive or cheat;
